What size hydration bladder does it hold?

The hydration bladder pouch should be able to house up to a 3 L/ 96 oz bladder.

How does one know what size to order? I'm 5'11", medium build. Does size depend on the distance from the bottom of the spine to bottom of the neck?

That is correct. you measure from your C7 vertebrae (the bump at the base of your neck) down to your hip crest. You can then view the sizing chart to select the appropriate size.
