Sounds like you would need an XS, but most of the XS harnesses start with waste around 23 inches. You could also look into a kid-specific harness like the BD WizKid, or a fully adjustable harness like the ABC student harness (http://www.campsaver.com/student-harness). The latter fits waist sizes from 20" to 42", it is surprisingly comfortable in spite of the lack of padding.
