I am a small waisted person and the small is a pretty good size. I have long legs so the length of the legs could be longer- they scootch right up to my butt (I'd like to be able to keep the legs tight without having them right next to my crotch- but it's not uncomfortable). Been out with this harness three times now and it is very comfortable. I have received comments of the "oooh" and "ahhh" nature from my climbing buddies about its eye catching appearance. The best thing is it is barely noticeable it is so comfy.
I am 5'9" 165 and got a medium, had to return and get a large, fits good.
Sounds like you would need an XS, but most of the XS harnesses start with waste around 23 inches. You could also look into a kid-specific harness like the BD WizKid, or a fully adjustable harness like the ABC student harness (http://www.campsaver.com/student-harness).
